Mechanism of action
Sildenafil carries out its therapeutic action through a potent inhibitory action of phosphodiesterase type 5 (PDE5). It is an enzyme responsible for the degradation of cyclic GMP (cyclic guanosine monophosphate or GMPc), a compound involved in the relaxation of smooth muscle.
The process leading to an erection involves the release of nitric oxide (NO) in the corpora cavernosa of the penis during sexual stimulation. Nitrates will not be combined with sildenafil because of a risk of potentiation of the hypotensive effect (collapse and risk of death in the epectase). Likewise, the use of sildenafil in hypotensive patients may worsen hypotension.
A starting dose of 25 mg of sildenafil should be considered when co-administering CYP3A4 inhibitors, such as ketoconazole, itraconazole, erythromycin, clarithromycin, cimetidine, ritonavir and saquinavir.
What if Viagra doesn’t work?
Sometimes, Viagra (or other erection medications) does not work. However, in the vast majority of cases, it is simply a matter of readjusting the prescribed dose or reminding us of how these drugs work: duration and effect of Viagra, how long to take it before sex, at the start or middle of a meal, etc. In any case, you will need to see a doctor to determine the possible causes and readjust your treatment or how to take it.
